NCI Center for Cancer Research Selects Esprit Health’s eSphere Online
for Nursing Genetics and Genomics Survey

CHICAGO, ILL., August 17, 2007 Esprit Health, LLC a privately held company that specializes in technology for automating the clinical research process, announced today that it has signed an agreement with the Genetics Branch of the Center for Cancer Research of the National Cancer Institute NCI), Bethesda MD, to provide eSphere Online, an Internet based data capture tool and complete clinical research data management system.

“We are very excited to work with Kathleen Calzone, the Principal Investigator, on this very important project” noted Frank Pierce, CEO and President of Esprit Health.

In this initial project, a web-based survey will be pilot tested with National Cancer Institute (NCI) and NIH Clinical Center (CC) nurses to assess attitudes, practices, receptivity, confidence and competency of the nursing workforce integrating genetic and genomics information related to common multi-factorial diseases in preparation for a national nursing workforce survey.

Genetic and genomic science is redefining the understanding of the continuum of human health and illness. Therefore, recognition of genetics and genomics as a central science for health professional knowledge is essential. Because all diseases and conditions have a genetic or genomic component, options for care for all persons will increasingly include genetic and genomic information along the pathways of prevention, screening, diagnostics, prognostics, selection of treatment, and monitoring of treatment effectiveness. The clinical application of genetic and genomic knowledge has major implications for the entire nursing profession regardless of academic preparation, role or practice setting. Healthcare will increasingly demand that the registered nurse (RN) use genetic and genomic information and technology when providing care. These expectations have direct implications for RN preparatory curricula, as well as for the 2.9 million practicing nurses.

“By selecting eSphere Online for their Internet based survey of nurses, the Center for Cancer Research will be able to quickly collect and then analyze their survey data, and ultimately decide on whether or not to proceed with their national nursing survey,” said Pierce.

eSphere, the company’s flagship product provides the most advanced, innovative methodology for clinical study management is now integrated with eSphere Online a web-based tool for data collection. NIH Clinical Center Adopts Esprit Health’s eSphere to provide
informatics support for Bereavement Program

CHICAGO, ILL., January 16, 2007 Esprit Health, LLC a privately held company that specializes in technology for automating the clinical research process, announced today that it has signed an agreement with the National Institute of Health Clinical Center, Bethesda MD, to utilize eSphere , a complete clinical research data management system for the Clinical Center’s Bereavement Program.

Patients and their families often describe CC physicians and nurses as honorary members of the family. And when a patient dies—either while at the Clinical Center or after leaving treatment—those relationships between CC staff and patients’ families do not end, or at least they shouldn’t, according to the CC Pain and Palliative Care Service (PPCS) team. “Families deserve that connection,” says Dr. Ann Berger, PPCS chief. “That emotional need is not over for the family the day the patient dies—and it shouldn’t be for us either.” As a means of continuing that service and support for families and staff, PPCS recently implemented a bereavement program. “Bereavement is part of palliative care’s mission,” she says.

Since its inception five years ago, PPCS has worked with resources all over the Clinical Center, from social workers and volunteers to therapists with the recreation department—to improve the quality of life for CC patients. After noting the increasing need for a formal bereavement program, Berger adopted the cause as one of her team’s top priorities.

When a patient dies, a physician, nurse or social worker notifies the bereavement program liaison. The liaison then consults with the patient’s attending physician to determine whether the family would respond well to bereavement support. Once the family is placed in the program, a volunteer arranges for a card to be signed by everyone who cared for the patient and sends it to the family. At one month, six months, one year and 13 months after the death, families receive a phone call from a volunteer. If there seems to be a risk of complicated grief, the volunteer will try to help the family member find a bereavement support group in the local community.

eSphere, provides the PPCS with innovative methodology to track bereavement families, develop a customized follow-up program, provide periodic reminders to program staff and volunteers on who to call and when and monitor the overall program. With a workflow process designed collaboratively by Esprit health and the PPCS, eSphere will help monitor every step in the process and provide management with statistical reports on the program’s effectiveness.

NIDDK Picks Esprit Health’s eSphere for Sleep Study

CHICAGO, ILL., June 16, 2006 Esprit Health, LLC a privately held company that specializes in technology for automating the clinical research process, announced today that it has signed an agreement with the Clinical Endocrinology Branch of the National Institute of Diabetes and Digestive & Kidney Diseases (NIDDK), Bethesda MD, to provide eSphere, a complete clinical research data management system.

eSphere, the company’s flagship product provides the most advanced, innovative methodology for clinical study management. It is a complete end-to-end system designed to address 100% of the clinical, research, regulatory and administrative processes and associated costs and provides all of the components necessary for a “true” clinical research data management system.

“We are very proud to have been selected as the solution for the Clinical Endocrinology Branch’s study on chronic sleep deprivation as a risk factor for metabolic syndrome and obesity noted Frank Pierce, CEO and President of Esprit Health. “By selecting eSphere for their system, the NIDDK will be giving its Principal and Associate Investigators, Research Nurses, and Administrators access to information that will help analyze clinical trial data, increase efficiencies, better understand resource utilization and costs and ultimately produce better outcomes.”

Obesity and chronic sleep deprivation have both become increasingly pervasive medical problems in recent years. The prevalence of adult obesity has doubled over the past 30 years and continues to increase. In addition, industrial societies attach an economic value to maximizing the waking period to the longest tolerable limit by sleeping as little as possible. Average sleep time has decreased over the last century by 2 hours. Chronically sleeping less has been associated with increased weight, endocrine and metabolic health risks including glucose intolerance, cardiovascular disease, and mortality. The possibility that the current epidemic of obesity and metabolic health risks may be partially related to insufficient sleep is now being recognized. The objective of this proof-of-concept controlled trial is to investigate the impact of increasing sleep time in chronically sleep-deprived, obese subjects.

NHGRI Picks Esprit Health's eSphere for Internet based Survey to Family Physicians

CHICAGO, ILL., August 31, 2004Esprit Health, LLC a privately held company that specializes in technology for automating the clinical research process, announced today that it has signed an agreement with the National Human Genome Research Institute (NHGRI), Bethesda MD, to provide eSphere, a complete clinical research data management system.

“We are very excited to work with Dr. Colleen McBride, Chief of the Social and Behavioral Research Branch of the NHGRI on this very important project” noted Frank Pierce, CEO and President of Esprit Health.

Current trends suggest that genetics will become increasingly relevant to and integrated into family medicine.  In recognition of these emerging genomic innovations and their potential for improving primary care practice, the American Academy of Family Physicians (AAFP),in collaboration with the National Human Genome Research Institute (NHGRI)has made genomic medicine the topic of their 2005 Annual Clinical Focus curriculum.   The AAFP is one of the largest medical organizations with a membership that comprises a group of 56,400 active family physicians.  This study seeks to collect data on family physicians’ attitudes and practices related to the genetics of complex disorders.  This information could be useful in understanding and shaping the dissemination of genetic medicine in ways that improve the standard of primary care practice.

In the first phase, a web-based survey will be conducted of members of AAFP.  In the second phase, a sub-sample of those who complete the survey will be re-contacted; half will be members who have enrolled in a year-long web-based curriculum related to medical genetics, and the other half will be those who have chosen not to enroll in the curriculum.

“By selecting our newest tool, eSphere Online for their Internet based survey of AAFP physicians, the NHGRI will be able to quickly collect and then analyze their survey data, and ultimately produce better outcomes,” said Pierce.

eSphere, the company’s flagship product provides the most advanced, innovative methodology for clinical study management is now integrated with eSphere Online a web-based tool for data collection.  Together they form a complete end-to-end system designed to address 100% of the clinical, research, regulatory and administrative processes and associated costs and provides all of the components necessary for a “true” clinical research data management system.

Esprit Health Releases White Papers on Business and Clinical Transformation

CHICAGO, ILL., July 2, 2004 – Esprit Health, LLC announced today that it has completed two research papers on the subject of Business and Clinical Transformation.  The papers thoroughly explore the fundamental need for medical care workflow-centric information systems that support structured approaches to medical care, and present the Esprit Health solution.

The first white paper, entitled “The Need for Radical Change” examines the forces that impact healthcare and provides insight into the problems inherent in the current healthcare delivery system and the supporting information technologies.  The “Protocol Centric Approach – A new Paradigm” is the second in the series and presents a framework for the healthcare organization to create the infrastructure to evolve to a “Protocol Centric” organization.   In the near future, Esprit will deliver its final research on the implementation effort required to become protocol centric.

The Esprit white papers conclude that of the many forces that healthcare organizations today must deal with, perhaps the two most significant are the public focus on the urgent need to improve patient safety and the financial pressures of managed care.  These two converging forces have made healthcare organizations especially aware of the need to implement key changes in the way care is delivered, to provide safer, more cost effective care to their patients.

Traditional responses of process improvement combined with some spending on information technology only perpetuates the current system and provide modest benefits, if any.  As if the computer revolution never happened, more than thirty years after the introduction of technology in healthcare, the core processes are still highly dependent on manual, paper-based processes and care delivery relies heavily on the memory of clinicians.  Financial pressures placed on the healthcare organization by managed care compound the problem.  In an industry where the largest cost component is labor, staffing reductions to control costs often eliminates the very resources utilized to monitor and improve the process.

Clearly, the current systems cannot do the job, and every attempt to improve it will produce the same sub-optimal results.  Each study and report on healthcare quality and safety underscores that the problem is the system.  Nothing short of transformative, fundamental change will address the problem.

The white papers in their entirety are available on the Esprit Health Web site at www.esprithealth.com

Esprit Health Discusses Solutions for Patient Logistics at 17th Annual IEEE Meeting

CHICAGO, ILL., May 1, 2004 –   Esprit Health has been selected to present its unique approach for “Transforming the scientific protocol document to meet the day-to-day operational needs of an active research protocol” to the symposium attendees.

This paper addresses the logistics of launching and managing the clinical research study.  Many times the details of the logistics of the day-to-day operations of the clinical study are not as well-thought out as the scientific question, the analyses and the research activity.  The operational aspects of the study can be complex, and must coordinate subjects, clinicians and equipment and facility resources.

Researchers operate within an environment in which the data collection process reflects the large number of complex, intricate and informal processes that are conducted by many different clinicians.  The resulting data to be managed is defined in the worksheets, flow sheets, questionnaires, surveys, assessments and other tools that these clinicians use.  The data collection effort associated with a protocol is in reality, an accumulation of data in a variety of forms (e.g. paper, spreadsheets, and databases) and formats.

Research data can be characterized as coming from multiple people, multiple sources, in multiple forms and formats, and retained in multiple databases.  Once gathered all of this disparate must be integrated in some fashion, a process that often leads to data inaccuracies, lost or misplaced data, and data corruption.

This paper describes experiences participating in two major clinical studies and presents an approach which was utilized in both that enabled clinical researchers to accelerate analysis and publish results sooner.  An informatics solution will be presented which facilitated patient management and coordination across the continuum of protocols.

Esprit Health Discusses Protocol Centric Approach at IEEE Meeting

CHICAGO, ILL., July 23, 2001 –  Esprit Health has been chosen to present its new paradigm for data management called the “procol centric approach.  The protocol-centric approach brings knowledge based decision support and best practices to the healthcare enterprise.  It is based on:

·         All work processes can be modeled – they are a series of activities, sequenced over time, which may have dependent relationships, and incorporates branching decision points leading to a desired outcome.

·         The smaller the process the more consistent or standard it becomes.

·         The “true” source of all data is the patient, the patient’s protocol(s), and the unplanned problems that arise from their interaction.

·         A single, longitudinal patient record must integrate all data across the continuum.

·         Healthcare is multidisciplinary and requires the ability to view health information from different perspectives.

This new paradigm seamlessly links a complex array of processes into a logical flow allowing the organization to become extremely efficient, achieve higher productivity and lower costs. Care delivery flows from the protocol as orders are initiated based upon it, and documentation is by exception.  The ability to look at problems and outcomes promotes understanding and improves quality.

At this weeks meeting of the 14th Annual IEEE Symposium on Computer Based Medical Systems, the President and CEO of Esprit Health, Frank Pierce will be participating and speaking at a session.  He will introduce the protocol-centric approach to healthcare information technology, a new approach in medical systems.  Pierce will present a technical paper defining the approach, present conceptual models and discuss the benefits of embracing this new paradigm.

ESPRIT HEALTH INTRODUCES NEW eSphere RELEASE

CHICAGO, ILL., April 7, 2001 – Esprit Health, LLC today announced the general availability of the newest major release eSphere, its innovative protocol-centric data collection system for healthcare.  Version 1.4, the new release, features more than 60 new features and enhancements ranging from system improvements to new software modules.  Several noteworthy new features are significant:

Access to Your Data – The eSphere Data Extract Wizard

eSphere now provides the ability to easily perform ad-hoc queries of the database without any programming expertise.  The eSphere Extract Wizard enables selection of one or more protocols followed by the identification of parameters (e.g. dates, demographic filters, etc) and selection of Care Modules and or specific items/outcomes.  Data is quickly extracted and exported to an Excel spreadsheet.  Once exported, the data can then be imported into any other tool for sophisticated reporting and statistical analyses.

From there data can be saved to any one of a number of standard formats such as standard CSV delimited file, or tab delimited file.

Observation Note

This new class of Items is similar in nature to PRNs in that they do not “appear” (i.e. scheduled at specific times or intervals) in the chart unless selected from the PRN window.

However, unlike PRNs which must be in the patient’s Plan of Care, items that are set up as an Observation Note do NOT need to be scheduled in the Plan of Care.  They can be inserted into the chart at any time and are available to all patients regardless of their pathway or individualized plan of care.

Extended Management Reporting.

eSphere now meets the goal of blending standard reports with ad-hoc query capability.  The system’s Personal Portal acts as an Internet based gateway to a rich array of data views and analyses that are customizable and extendable.   utilizing the customized Personal Portal as the gateway to the research data.  Among the “standard” reports are:

·         Outcome Analyses

·         Level of Criticality reports

·         Pre-defined data views based on the requirements and specifications defined by the individual user logging on (e.g. physician)

Other Important Enhancements and Improvements

As with every major release and every subsequent build of eSphere, there are always numerous system enhancements and improvements being added on a daily basis!  Some of the new enhancements already included are:

·         Data column sorting

·         Improved Version control at data element level
